Duration: 60 minutes Course Description Medicare pays for the care your team already provides between office visits, but many practices leave that revenue on the table or bill these services in ways that put them at risk. Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) are two of the most valuable programs available to primary care and specialty practices today, and 2026 brought some of the biggest changes since these codes were introduced. Starting January 1, 2026, CMS added new RPM codes for patients who submit only 2 to 15 days of readings, as well as a new code for the first 10 minutes of monthly management. Advanced Primary Care Management (APCM) now includes behavioral health add-on codes, and FQHCs and RHCs can now bill care management services using individual codes. Practices that do not update their workflows may miss revenue opportunities and face claim denials. About the Presenter Dr. Irina Koyfman, DNP, NP-C, RN, is the President and CEO of Affinity Expert, a company focused on chronic care management, care coordination, and related healthcare services. She brings more than 25 years of clinical and executive experience and leads by a simple principle: Purpose Before Profit. Dr. Koyfman is a certified Adult Nurse Practitioner and Dementia Care Specialist. She earned her Bachelor of Science in Nursing from Villa Julie College, her Master of Science in Nursing as a Nurse Practitioner from the University of Maryland, and her Doctor of Nursing Practice with an executive focus from the Johns Hopkins School of Nursing. She continues to care for homebound patients and has served as adjunct faculty at several universities. She is a member of the American Academy of Nurse Practitioners and the Gerontological Advanced Practice Nurses Association. She has also served on community and nonprofit boards, including the Board of Trustees of the Community College of Baltimore County.
